Non-Athletes: Complete a non-athlete membership form. If this form does not print properly, or you wish to print out multiple copies of the form, use the form titled "Non-Athlete Membership App" found in the Downloads section.

Print out the completed non-athlete application from your local printer.

Send the completed form and a check or money order payable to United States Diving, 201 S. Capitol Ave., Ste. 430, Indianapolis, IN 46225 USA. Sorry, we cannot accept credit cards.

If you are not an active diver, skip step 4.
Athletes: Do not fill out the non-athlete membership form. Contact your local registration chairperson for information on how to join in your area. A list of registration chairs, including their address and telephone, can be found in the Membership section.
